2. NRIC/Passport Number, and
3. Complete Correspondence Address.

Corporate Donors:

1. Company Name,
2. Company Registration Number, and
3. Registered/Business Address

All personal and corporate details provided will be kept strictly confidential and securely protected in accordance with data protection laws.
